GHCMyChart Terms and Conditions of Use

SIGN UP AND USE OF GHCMYCHART DENOTES YOUR AGREEMENT TO COMPLY WITH THESE TERMS AND CONDITIONS OF USE. IF YOU DO NOT AGREE WITH THE TERMS AND CONDITIONS, DO NOT PROCEED WITH SIGN-UP OR USE OF THIS SYSTEM.
What is GHCMyChart?
GHCMyChart is a web-based version of our electronic record system that provides you with secure access to your health and insurance information. GHC-SCW is not required to offer our members GHCMyChart but we do provide it as an added feature designed to enhance your care and treatment. GHCMyChart does not contain a complete copy of your medical or insurance record and it does not replace or eliminate the need for you to contact GHC-SCW via telephone or in person. In some cases, you may still have to submit requests for information using conventional paper processes. Additionally, GHC-SCW may not be able to provide you with certain information due to the limitations of electronic technology available in this product.
Privacy of Your Information
One of the most important fundamental principles that guide GHC-SCW’s care, treatment, and services, as well as in our selection of technology, is confidence and assurance that this sensitive information is collected, used, disclosed and transmitted in a private and secure manner. It matters to you, and it matters to GHC-SCW. You can be assured that your confidential health and insurance information is handled in accordance with all applicable federal and state laws governing these processes. Our Notice of Privacy Practices describes in detail your rights and GHC-SCW responsibilities as they relate to the processing of protected health information (PHI). One of the key provisions in the GHC-SCW Notice of Privacy Practices is clarification that your health and insurance information will be disclosed only as permitted in the Notice of Privacy Practices or with your written authorization.
Security of Your Information
Firewalls, multifactor authentication, encryption, and audit trails are some of the tools we use to make sure that your information is not only private, but it’s secure. If the URL begins with https:// (instead of http://), the document comes to you from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. Further, you can tell whether you are connected to GHC-SCW by viewing the digital certificate which verifies the connection between the GHC-SCW server’s public key and the server’s identification. For other general information viewing, GHCMyChart must be accessed with a Transport Layer Security (TLS) compatible browser or terminal (Google Chrome or Microsoft Edge). Our TLS web server uses authentication and offers the highest level of encryption technology commercially available (256-bit AES). In addition, user names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. There are also some things you can do to ensure security in GHCMyChart. We urge you to change your password on a regular basis and keep your login ID and password confidential, as well as enrolling your account in Multifactor Authentication. Multifactor Authentication allows you to attach your phone number to your GHCMyChart login and verify that you are the person accessing your information, adding an extra layer of security protecting your information.

You should be aware that you will be notified via e-mail when there is new medical information to be viewed. This means any person with access to your e-mail account associated with your GHCMyChart account will be able to see this notification. This could include your spouse, employer, parent, or anyone else that can access your e-mail account. Although no private medical information will be in the message, the notification that new medical information is available may itself be information that you do not want others to know. You should take this into account when providing your e-mail address.
GHC-SCW may send you e-mail messages that contain information important to your personal health care and treatment. It is your responsibility to monitor these messages. GHC-SCW may not be held responsible or liable for any loss, injury, claims or any other adverse effect resulting from your failure to read these messages in a timely manner. The anticipated turnaround time for a response to your electronic messages is three (3) business days. GHCMyChart messages and information will be stored in your GHC-SCW medical record and accessed, reviewed and processed by any GHC-SCW employee involved in your care treatment, payment and health care operations. You are responsible for maintaining your username and password since this information will be used to identify you in GHCMyChart and any inquiries or communications that you make will be verified with your identity at login.

GHCMyChart and Me
GHCMyChart is not a complete copy of your medical record and it should not be used as a substitute for ongoing care and treatment with your health care provider. The following points describe important clarifications that you must acknowledge and understand with respect to your use of GHCMyChart:
Termination of Access
I understand and acknowledge that my GHCMyChart access may be restricted or revoked at any time, for any reason, in GHC-SCW’s sole discretion. Circ*mstances and/or behaviors that may result in restriction or revocation of my access to GHCMyChart include, but are not limited to:
If a user’s access to GHCMyChart is restricted or revoked for any reason, GHC-SCW will make every reasonable effort to notify the user but it is your responsibility to ensure continuity of care and treatment if there are outstanding issues being handled in GHCMyChart. If a restriction or revocation is related to GHC-SCW receiving applicable information through another venue, such as court documentation or implementation of a policy unrelated to GHCMyChart, access may be revoked or restricted accordingly.
GHC-SCW may restrict or revoke proxy access to any account at any time, for any reason, in GHC-SCW’s sole discretion.
Revocation or restriction of access to GHCMyChart does not constitute a restriction on accessing PHI to which you have a legal right of access. For more information regarding your rights to access PHI, please contact our Privacy Officer at privacy@ghcscw.com or (608) 662-4899.
GHCMyChart Eligibility
Any qualifying GHC-SCW member, age 18 or over, may request a personal GHCMyChart account.
Any GHC-SCW member under the age of 18 may request a personal GHCMyChart account with the written consent of their parent or legal guardian.
Proxy Access to GHCMyChart
The parent or legal guardian of a minor under the age of 18 may request access to the Minor’s GHCMyChart account. In order to be granted proxy access, the parent or legal guardian must also have a GHCMyChart account in place. There are several levels of proxy access at GHC-SCW:
Proxy Access related to Patients under the age of 12
Parents and legal guardians can send messages to the child's providers, schedule appointments, view upcoming and past appointments, pay bills, view immunization records, test results and more.
Proxy Access realted to Patients age 12-17
When a child turns 12 years old, the proxy access of parents or legal guardians is automatically adjusted to recognize the privacy rights of adolescents. Under State and Federal law, certain types of medical information either requires the patient’s consent to be shared, or the patient may have the right to request confidentiality. In recognition of those legal rights, the full functionality of GHCMyChart services for a proxy may be limited. Proxies continue to have access to send messages to the care team, view immunizations, pay bills, refill most medications, and review most test results. Other Proxy Access Information
USE OF THIRD-PARTY SERVICES
You may choose to use other services provided by third parties in conjunction with your use of GHCMyChart. Such services may include, but are not limited to, translation services, location services, fitness tracking, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor GHC-SCW have control over the contents of these services, and neither Epic nor GHC-SCW accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.
