Terms and Conditions of Use

Welcome to MyChart!

What is MyChart?

MyChart is an internet application (“Service”) that enables a patient to have secure web-based access to portions of his/her medical record and allows a secure, convenient communications channel with United Regional for non-urgent messages. Acceptance of Terms and Conditions is required to access your medical information online. Use of this service is optional. By accessing or using MyChart, you agree that you have read, understand, and agree to be bound by these Terms and Conditions, as amended from time to time. Your continued use of the service after the date of amended Terms and Conditions will constitute your acceptance. If you do not agree to the Terms and Conditions when amended, you may stop using the service and cancel your account. The terms "United Regional," "we," and "our" refer to United Regional Health Care System, including the hospital, and United Regional Physician Group.

Notice of Health Information Privacy Practices

United Regional considers the privacy of your health information to be an important element in our relationship with you. Our responsibility to maintain the confidentiality of your health information is one that we take very seriously. The United Regional Privacy Policy as posted and amended from time to time is hereby incorporated into these Terms and Conditions.By agreeing to these Terms and Conditions, I acknowledge that I am requesting access to portions of my health information and the ability to communicate with my United Regional health care team concerning my health information via the internet using an electronic application called MyChart.

Use of MyChart for Health Care Services

I understand that MyChart should never be used for urgent matters. For all urgent matters that I believe may immediately affect my health or well-being, I WILL DIAL 911 AND/OR GO TO A LOCAL EMERGENCY ROOM.

I understand that my United Regional health care team may send me messages via MyChart. These messages may contain information that is important to my health and medical care. It is my responsibility to monitor these messages that are sent to my MyChart Inbox. I will maintain a valid and functional email address that may be used to notify me of the presence of MyChart messages. I will provide this email address for notification of the presence of MyChart messages and I will update my email address as needed. I agree not to hold United Regional liable for any loss, injury or claims of any kind resulting messages that I fail to read in a timely manner.
I agree that all communication through MyChart will be in regard to my own health condition(s). I understand that contents of any message may be stored in my permanent medical record. I understand that asking for advice on behalf of another person could potentially be harmful and is a violation of the MyChart terms. The United Regional health care team does not assume any responsibility for health information or services used by persons other than the MyChart enrollee.

Verification of Identity

I understand that my enrollment is contingent on verification of my identity by United Regional. Patients who wish to participate will be issued a MyChart activation code during their visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call 940-764-HELP (4357) to get one or ask to sign up during your next office visit.

Minor Eligibility

Unless otherwise allowed by law, an individual must be age 18 or older to request a MyChart account. If allowed by law, Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section. Under state and federal law, there may be circ*mstances where you will not be allowed access to your child's health information. Because of these legal requirements, United Regional reserves the right to deny you access as appropriate under the law.

Proxy Access

Individuals may request proxy access to another individual’s MyChart account by completing the Proxy Access portion of the MyChart Consent Form. Such access will only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information, or if the patient provides written authorization. Only one person will be granted proxy access to a MyChart account. A MyChart account will be activated for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must inform United Regional immediately by sending written notice to:

MyChart ID and Password

I understand that I will create a unique identification (ID) code and password to be used to access my health information via MyChart. I understand that this ID and password are unique codes that identify me in the MyChart computer system. Inquiries and entries that I make via United Regional will be logged with my identity.
I understand that it is extremely important that I keep the ID and password that I use to access MyChart completely confidential. If at any time I feel that the confidentiality of my password has been compromised, I will change it by going to the Password link on the MyChart website. I understand that United Regional takes no responsibility for and disclaim any and all liability or consequential damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password. If United Regional discovers that I have inappropriately shared my password with another person, or that I have misused or abused my MyChart access privileges in any way, my participation in MyChart may be discontinued by the health care team without prior notice.

Notice of Computer and Network Security

You are attempting to obtain access to a component of the computer resources of United Regional. These resources, including all related equipment, networks, and network devices (specifically including internet access), are provided only for authorized use. All data residing on any component of this system is the property of United Regional and is protected by state and federal laws and regulations.
United Regional computer resources may be monitored for all lawful purposes, including to ensure that their use is authorized, for system management, to facilitate protection against unauthorized access, and to verify security procedures, survivability, and operational security. Monitoring includes security testing by authorized entities to test or verify the security of this system. All information, including personal information, placed on or sent over this system may be used for authorized purposes only.
Unauthorized use may subject you to criminal prosecution. Evidence of unauthorized use may be used for administrative, criminal or other disciplinary action. Use of this computer system, authorized or unauthorized, constitutes consent to monitoring of this system.

Voluntary Deactivation of MyChart

You can voluntarily disenroll from your use of MyChart at any time by completing the appropriate online request to disenroll or by calling 940-764-HELP (4357).

Involuntary Deactivation of MyChart

Access to MyChart is a courtesy and not a patient right. United Regional reserves the right in its discretion to discontinue your access. The following is a list of reasons which may result in deactivation. The list is not inclusive and each circ*mstance will be evaluated individually.

  • Automatic system deactivation will occur for a parent to view a child’s record when the child reaches the age of 18
  • Inappropriate use of MyChart (examples include: excessive, unnecessary, inappropriate content in messages or use of scheduling)
  • Abuse of proxy access
  • Pending legal issues with patient or direct family

You will be notified if we deactivate your MyChart access. You may obtain a copy of your medical records by sending a written request to the medical records department:

Use of Third-Party Services

You may choose to use other services provided by third parties in conjunction with your use of MyChart. MyChart may offer links related to medical web sites not managed by United Regional. United Regional receives no payment or other remuneration in exchange for your linking to or using any third party site. Such services may include, but are not limited to, patient informational services, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor United Regional has control over the contents of these services, and neither Epic nor United Regional accepts any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

Surveys and Use of Information

I understand that from time to time I may be asked to complete patient satisfaction surveys via MyChart. United Regional may analyze any information submitted via MyChart as part of descriptive (demographic) studies and reports. By submitting information through the Service, you provide United Regional the authorization to use and distribute it only as permitted by applicable law.

Disclaimer

I understand that MyChart may not be available to me all the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of United Regional. Access is provided on an "as-is, as-available" basis and does not guarantee that I will be able to access MyChart at any particular time.
I UNDERSTAND THAT UNITED REGIONAL TAKES NO RESPONSIBILITY FOR AND DISCLAIMS ANY AND ALL LIABILITY ARISING FROM ANY INACCURACIES OR DEFECTS IN SOFTWARE, COMMUNICATION LINES, VIRTUAL PRIVATE NETWORK, THE INTERNET OR MY INTERNET SERVICE PROVIDER (ISP), ACCESS SYSTEM, COMPUTER HARDWARE OR SOFTWARE, OR ANY OTHER SERVICE OR DEVICE THAT I USE TO ACCESS MYCHART. To the fullest extent of the law, you expressly agree to fully waive, release and discharge United Regional, its entities, affiliates, physicians, physician practices, providers or any of its staff from any and all claims, liability damages and/or losses. These Terms and Conditions are governed by Texas law, with the state and federal courts located in Wichita County.

During times when MyChart is unavailable, other communication methods (e.g., telephone) should be used to access United Regional.
