How to get medical records from University of Louisville Hospital? ›

If you would like to access your medical records, please call 502-562-3062 or 502-217-1900. You may download and print our Release of Information Form to take with you when you pick up your records. Release of Information Forms: English.

Kentucky. Kentucky medical facilities must keep patient records for at least six years following the final date of discharge. Individual doctor regulations do not exist in this state.

UofL Hospital's Trauma Center is verified as a Level I trauma center by the Verification Review Committee (VRC), an ad hoc committee of the Committee on Trauma (COT) of the American College of Surgeons (ACS). This achievement recognizes University Hospital's dedication to providing optimal care for injured patients.

You may request your records by filling out an authorization form in person at our new off-campus location, 2333 Alumni Park Plaza, Suite 110, Lexington, KY 40517, or our campus location on the 1st floor of Chandler (PAV H), next to the registration desk. You may call for the form to get your records.

Please download and complete the authorization form to submit your medical record request by fax, email or mail. Verification of identity may be required. Acceptable forms of identification include driver's license, employment ID, state-issued ID, current school ID, military ID, VA. ID or a valid, current passport.

In addition, two categories of information are expressly excluded from the right of access: Psychotherapy notes, which are the personal notes of a mental health care provider documenting or analyzing the contents of a counseling session, that are maintained separate from the rest of the patient's medical record.

A prescription is not considered to be part of the medical record.

A copying fee, not to exceed one dollar ($1) per page, may be charged by the health care provider for furnishing a second copy of the patient's medical record upon request either by the patient or the patient's attorney or the patient's authorized representative.
